The error, which occurred at Grange University Hospital in Cwmbran, was uncovered three weeks following the initial service, leading to a second funeral for the correct deceased.
The Aneurin Bevan University Health Board has expressed deep regret for the distress caused by the mishap, which took place at Sirhowy Valley Crematorium, and has offered its full support to the affected family, ensuring the public that this was a rare and self-discovered human error. The Health Board has pledged to keep the family informed throughout their internal investigation.
The crematorium involved, part of the Westerleigh Group, declined to comment out of respect for family privacy, except to affirm their commitment to exceptional care and support.
