Vennells allegedly failed to disclose critical information about software bugs and the reliability of key witness Gareth Jenkins.
A memo by barrister Simon Clarke KC, which highlighted bugs in the Horizon system, was reportedly not shown to Vennells by Post Office lawyers.
Swinson expressed regret for not doing more during her tenure, becoming emotional while addressing sub-postmasters at the inquiry.
