This mishap occurred in November 2023 and was similar to another incident that took place in December of the same year. The organization expressed profound regret to the affected family and attributed the mistake to human error.

WalesOnline reported that the family learned about the mix-up several months later. Following an initial investigation, the health board is now examining the details of the second error. Measures have been taken to prevent further occurrences, with additional checks implemented in the mortuary.

The board has conducted a comprehensive review to verify that no other errors occurred and has met with the grieving family to offer support. Due to the unavailability of relatives for the other deceased individual, there has been a delay in bringing the second case to light.

The health board has vowed to keep the family informed about the ongoing inquiry and reiterated its sympathy and unwavering support for them during this difficult time.
