Former Post Office chair Alice Perkins has accused senior executives of misleading the board about issues with the Horizon computer system, which caused the largest miscarriage of justice in British history.

Perkins, who chaired the Post Office from 2011 to 2015, told an inquiry that she did not cover up Horizon's issues.

Despite warnings, the board was often given incomplete and misleading information.

Much of her tenure overlapped with CEO Paula Vennells, who has also faced scrutiny.

Perkins expressed regret for not uncovering the truth sooner.
